Simply complete Form 4642 (called the Michigan Voluntary Contributions and Anatomical Gift Donor Registry Schedule) and include it with your other tax documents. Download it here for your convenience and share with your tax preparer:
The American Red Cross Michigan Fund is listed first on Form 4642. Check the appropriate box to donate $5 (Column A), or $10 (Column B), or enter an amount of your choosing (in Column C). Be sure to carry the amount of your contribution from Column D on this form to Line 22 on your MI-1040 form and include the Form 4642 with your other tax forms.
